Leaving the Hospital

While you are in the hospital, your care team will carefully evaluate your progress to determine the best setting for you after you leave. You probably will be able to go straight home if you have family or a health care aide to help you. If your team feels that it is necessary, they may suggest a short stay in a nursing home, short-term care facility, or rehabilitation facility. These decisions are made together with you and your family. Either way, you will continue to receive comprehensive care, which includes therapy.

Outpatient therapy can be provided in your home at first, and can then be continued at one of several convenient community locations.

The sooner you seek treatment, the better you'll feel.
